Referral Instructions for non-emergent outpatient referrals only.

1.  Please fax completed referral form and records to (309)634-8884.

2.  Complete records are essential in determining the urgecy of referrals.  records to be faxed should include the following:         

     >Growth Chart
     >Any workups you have completed on the patient prior to this referral.

3.  A medical assistant or nurse will contact your office after the physician has reveiwed the records.  Please expect a call within 3 days.

4.  To expedite treatment, we may ask your office for additional tests to be ordered prior to the consultation appointment.
